SUMMARY

Provides primary care medical services to the patients of UMMA.

ESSENTIAL DUTIES

  • Clinical Care:
    • Provide comprehensive, patient-centered primary care for pediatric patients, including well-child visits, acute care, and chronic disease management.
    • Conduct developmental screenings, immunizations, and preventive health services.
    • Educate families on preventive care, chronic disease management, and health promotion strategies.
    • Collaborate with pediatricians and an interdisciplinary care team to ensure optimal patient outcomes.
    • Maintain current knowledge-base and appropriate licensure
    • Able to complete patient charting in a timely fashion to support billing levels.
    • Able to address all inbox messages in a timely manner
  • Quality Improvement & Metrics:

    • Lead and participate in QI initiatives aimed at improving pediatric health outcomes, including vaccination rates, developmental screenings, asthma control, and well-child visit adherence.
    • Utilize data analytics and electronic health records (EHR) to track quality metrics and identify areas for improvement.
    • Implement and evaluate evidence-based interventions to enhance patient care and health equity.
    • Work closely with the Quality Improvement team to ensure adherence to national and state performance measures, including HEDIS and other national evidence based standards.
  • Collaboration & Leadership:

    • Partner with pediatricians, nurses, and care coordinators to develop workflow improvements that enhance efficiency and patient experience.
    • Provide training and mentorship to clinical staff on best practices for achieving pediatric quality benchmarks.
    • Engage in community outreach efforts to improve access to pediatric care and preventive services.
    • Able to work 1 Saturday a month if need is identified
    • Active participation not only in Quality Improvement Program, but also in Peer Review, Provider and clinical meetings, as well as UMMA's Coverage and On-call schedule.

SUPERVISORY RESPONSIBILITY

This position may have supervisory responsibility to medical assistants and licensed vocational nurses. He or she may be expected to provide leadership and instruction to volunteers, students and less experienced staff.

QUALIFICATIONS/ POSITION REQUIREMENTS
  • Successful completion and acceptable results of post offer physical examination, drug screening, and criminal background check.
  • Recommended Healthcare Personnel Requirements for vaccinations and preventative testing: Hep B, Influenza, MMR, TDAP, TB, Covid-19 and Booster.


EXPERIENCE/KNOWLEDGE:

To perform effectively in this position, the incumbent must have:

  • Certificate to practice as a Nurse Practitioner by the CA Board of Registered Nursing.
  • Active DEA license
  • Leadership and/or management skills preferred.
  • Spanish Language preferred

EDUCATION:
  • Masters or Doctoral Degree
  • Requires valid license to practice medicine in the State of California.
  • Board Certified or Board Eligible in specialty preferred.

PHYSICAL ACTIVITY

While performing this work, the incumbent:
  • Will spend substantial time standing, sitting, speaking and listening
  • Must regularly lift and/or move up to 10 pounds, and occasionally lift and/or move up to 25 pounds
  • Is expected to travel via personal vehicle throughout the community on a frequent basis
