Description

The County of San Mateo's Family Health Services Department is seeking a Pharmacist to join our team! Family Health Services (FHS) is a division within San Mateo County Health that houses the Immunization Program, as well as programs serving primarily pregnant people, parenting families, and children. This is a full-time, regular position.

The Pharmacist will be responsible for coordinating the Immunization Program and supporting partners across the County, including clinics, schools, childcare centers, and other health and social service organizations, in ensuring vaccine safety and efficacy is maintained and immunizations are available to all. This position requires a motivated individual with strong clinical skills and understanding of vaccines, as well as skills in critical thinking, data analysis, big-picture/system wide analysis, project management, computer system use, and oral/written communication.

The Pharmacist will be responsible for coordinating and executing the State Immunization Branch's workplan, including working closely with partners to maintain proper vaccine accountability and management, ensure there is broad access and high utilization of quality immunization services, use the California Immunization Registry and data reporting effectively, support education and training around vaccine safety and administration, and lead seasonal efforts supporting childcare and school immunization entry requirements and respiratory virus vaccination (Flu and COVID-19). The pharmacist should approach their role with a strong collaborative and learning mindset. The position will report to the FHS Immunization Program Manager.

Primary duties will include, but are not limited to:
  • Staying up to date on all Federal and State level immunization recommendations and guidance
  • Advise patients and partner providers of vaccine use, giving information regarding proper use, storage, common side effects, and drug interactions
  • Ensuring relevant providers and staff are properly trained on current guidance for safe vaccine management, storage and handling, and administration.
  • Conduct Quality Assurance verification of proper vaccine management and clinical safety, including supporting clinical staff and working with vaccine manufacturers in responding to vaccine management problems like temperature excursions
  • Supporting providers with enrollment in State and Federal immunization programs (including VFC, VFA/317, State General Fund Flu, and CA BAP) and compliance with State and Federal vaccine guidelines and program requirements
  • Estimate needs and order vaccines and relevant pharmaceuticals.
  • Maintain and keep in order vaccine and pharmaceutical supplies.
  • Supporting safe and efficient distribution of State and Federally supplied vaccine
  • Work with partners to improve equitable access to and receipt of all ACIP-recommended immunizations, especially for low income and underserved community members
  • Promoting and optimizing the use of the California Immunization Registry (CAIR) in the jurisdiction
  • Supporting partners in using web-based platforms like myCAvax, MyTurn, and CAIR Quick Entry for vaccine management and documentation
  • Reviewing CAIR data to help support partner's immunization efforts, including evaluating and adjusting strategies to reach target populations
  • Collaborate with the Communicable Disease team on any outbreak response requiring vaccines and as needed on efforts regarding reducing the incidence of perinatal hepatitis B virus
  • Participate in management of medical emergencies, adverse drug interactions or reactions, acute and chronic disease states.
  • Build and maintain close relationships with and disseminate relevant notices, print resources, and/or electronic communications among partner providers, schools, and other immunization stakeholders
  • Conduct and develop/update resources and materials for immunization related trainings with/for partners
  • Collaborate with schools and childcare centers within the jurisdiction on efforts to decrease the proportion of students who do not meet immunization requirements, including supporting schools and childcare centers in their annual immunization rate reporting
  • Coordinate and assist partners in planning and implementing seasonal respiratory virus vaccination efforts, including community-based seasonal respiratory virus vaccination clinics that target people with limited access to vaccines, and potentially administering vaccines if needed
  • Initiate and collaborate with partners on quality assurance projects and Pharmacy/hospital committees as needed
  • Communicate and collaborate with State Immunization Branch and other Local Health Jurisdictions including preparing reports on program activities as required.
  • Act as pharmacist-in-charge when so designated
  • Perform related duties as assigned

The ideal candidate will:
  • Have 2-3 years of experience with public health immunization programs, including emergency response
  • Possess knowledge of public health programming for individuals from diverse backgrounds and cultures, with an emphasis on vaccination and testing services
  • Be proficient with modern computing systems and web-based technologies and productivity software
  • Have experience exporting and manipulating data from vaccine clinic management and reporting systems, such as MyTurn, myCAvax, and CAIR
  • Be self-directed, take initiative, and be solutions-driven, with strong attention to detail
  • Proactively seek support and feedback and incorporate feedback into work products and approach
  • Have experience with improvement work
  • Demonstrate the ability to develop strong, collaborative working relationships with organizations and individuals from diverse backgrounds
  • Demonstrate the ability to work with partners to understand needs, including data and system-level needs, and design and build solutions to completion
  • Have excellent written and verbal communication skills

Qualifications

Knowledge of:
  • Prescription and non-prescription drugs and drug products and their indications, efficacy, dosage, mechanisms of action; fate and disposition (if known), major contraindications and potential side effects.
  • Influence of patient's age, sex, concomitant disease states, concurrent drug therapy, foods and diagnostic procedures on drug activity and disposition.
  • Physiology, anatomy, pathology, chemistry and mathematics as they relate to pharmacy practice.
  • Major reference sources to answer drug information/ingestion questions.
  • Pharmaceutical manufacturing methods.
  • Legislation governing pharmacy practice and hospital pharmacy services/hospital operations as they relate to pharmacy services.
Skill/Ability to:
  • Compound and manufacture a wide variety of medications.
  • Prepare IV medications.
  • Prepare unit dose medications.
  • Learn and carry out pharmacy procedures and practices.
  • Identify, achieve, maintain and use those competencies directly relevant to his/her area of practice.
  • Establish and maintain cooperative working relationships and provide functional direction to para-professional personnel.
  • Effectively counsel patients regarding use, possible side effects and storage of medications.
  • Effectively advise and consult with medical, nursing and other personnel regarding properties of drug substances.
  • Use and evaluate information in appropriate reference sources and clinical literature.
  • Maintain records and prepare reports as required by federal and state laws.
  • Communicate clearly and effectively.
  • Understand and carry out oral and written instructions.
  • Deal tactfully and work cooperatively with hospital staff and patients.
Licensure/Certification:
  • License to practice pharmacy in California.
