Description
Ensures hospital compliance with regulations from the Texas State Board of Pharmacy, CMS, and survey guidelines from CIHQ. Interprets physician prescriptions and medication orders. Acts as a drug information resource to patients, medical staff, nursing staff and ancillary department staff. Dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties.
Requirements
QUALIFICATIONS
Bachelor of Science degree or PharmD from ACPE accredited school of pharmacy
Licensure by the Texas State Board of Pharmacy is required
One (1) year of experience in a hospital setting preferred
Previous supervisory experience preferred
ROLE AND RESPONSIBILITIES
Performs job responsibilities in accordance with organization and departmental policies, procedures, performance improvement objectives, environmental and infection control standards, as well as state and federal laws and regulations
Performs computer order entry in accurate, thorough and timely manner. Reviews and updates medication profiles and is cognizant of patient charges
Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seeks clarification if necessary and documents interventions.
Participates in cost effective use and selection of pharmaceuticals.
Communicates drug information to physicians, nurses, etc. in courteous and professional manner.
Assist in patient and family education and counseling regarding drug therapy and side effects
Documents clinical activities, demonstrating cost savings.
Conveys key information required for follow-by fellow pharmacists
Manages technicians and other supportive personnel
Delegates appropriate and reasonable tasks and validates accuracy of floor stock replacements, cart fills and other drug doses dispensed from pharmacy
Audits and signs out all controlled substances in accordance with state and federal guidelines and organizational policies.
Investigates and corrects discrepancies and unusual usage patterns and trends
Participates in clinical program
Leads P&T initiatives and prepares pertinent P&T reports
Monitors target drugs for protocol discontinue, substitution, IV to PO, etc.
Collects applicable data and measures and communicates opportunities for improvement
Performs therapeutic monitoring of specified drug therapies and recommends dosage adjustments as needed based on renal function evaluation and pharmacokinetic calculations
Performs other duties as assigned within the scope of licensure.

Exposure to blood and body fluids, communicable diseases, chemicals, radiation, and repetitive motions
Light lifting 11-20 lbs.
Conditions include working inside; around machines, radiant and electrical energy; working closely with others or working alone; working protracted or irregular hours.
Position requires reaching, bending, stooping, and handling objects with hands and/or fingers, talking and/or hearing, and seeing
