Summary:
The Regional Pharmacy Clinical Coordinator is responsible for oversight of the provision of clinical pharmacy services. He/she will coordinate with the Corporate Director, Medication Use Management, and the multi-facility and facility pharmacy directors/managers within their region to ensure safe and effective drug therapy and optimal patient outcomes.
Essential functions/Areas of responsibility shall include the following:
Medication Use & Formulary Serves as non-voting member of system Pharmacy & Therapeutics (P&T) Committee. Monitors drug utilization reports or performs routine chart audits to evaluate compliance to institutional guidelines and initiatives. Facilitates medication use evaluations (MUE). Facilitates formulary reviews for drugs new to market, requests made by medical staff, or in response to changes in the safety, efficacy, or utilization affecting the formulary status of a medication or class. Reviews drug shortage databases and recommend alternative therapies when needed. Contributes to P&T newsletter content. Serves as a liaison to healthcare practitioners within the region to disseminate information regarding medication formulary changes. Reviews order sets and/or facilitate order set review by pharmacy subject matter experts. Serves on order set review oversight committee(s). Approves non-formulary requests from prescribers as appropriate.
Medication Stewardship & Medication Safety Serves on corporate, regional, and/or facility-level antimicrobial, anticoagulation, opioid, and other high-risk drug stewardship committees. Serves on corporate, regional, and/or facility-level medication safety committees. Coordinates stewardship and medication safety initiative efforts at all facilities within the region. Reviews medication errors/adverse drug events and report to P&T Committee and other committees as required.
Clinical Initiatives & Education Assists in development and implementation of clinical programs that match the needs of the patients and facility. Coordinates with Willow Information Technology to optimize clinical pharmacy electronic medical record (EMR) workflows. Leads education initiatives for facilities within the region that have been prioritized by the Corporate Director, Medication Use Management. Serves as a subject matter expert for required competencies. Provides drug information to pharmacy team members, prescribers, and other clinical staff as requested.
Residents & Students Serves as a Residency Program Director at a facility within the region. Oversees pharmacy team member and resident involvement with doctor of pharmacy students. Precepts assigned pharmacy student experiences.
Pharmacy Management Write, review, and maintain policies related to clinical services and the pharmacy residency programs. Participates in onboarding of new pharmacy team members as requested. Oversees and evaluates the work of clinical pharmacy staff as requested.
The Regional Pharmacy Clinical Coordinator shall model the Ballad Health Mission, Vision, and Values, and shall be committed, by personal actions/examples and leadership direction, to the unceasing improvement of care quality via the organization's Performance Improvement Initiatives. This pharmacist shall adhere to/serve as a role model as it relates to compliance with regulatory body/agency mandates. He/she will be knowledgeable about rationale and processes for accreditation.
Access to patient information will be needed to fulfill the responsibilities of this role. Adherence to Ballad Health confidentiality policies and practices is required.
INDCORP
Requirements:
The Regional Pharmacy Clinical Coordinator shall be a graduate of an ACPE-accredited Doctor of Pharmacy degree program. An applicable residency or comparable experience accompanied by Board certification from the Board of Pharmacy Specialties (BPS) shall be required. The Regional Pharmacy Clinical Coordinator shall have at least five (5) years of clinical pharmacy experience. Capabilities/knowledge must be exhibited in the use of related information systems hardware/software. Previous experience/proficiency in the provision of education and training for pharmacy students, pharmacy residents and healthcare providers are needed. This individual shall possess excellent communication / etiquette skills along with analytical and leadership abilities. He/she must demonstrate the ability to work closely with physicians and other members of the healthcare team in an interprofessional setting.
Licenses and Certifications:
Valid and active pharmacist licensure in appropriate state(s)
BLS required, ACLS and/or PALS may be required based upon assigned job duties Board of Pharmaceutical Specialties Certification in at least one discipline.
