Position Duties and Responsibilities:
- HCW shall provide services within the scope of the clinical privileges granted by the MTF Director and the applicable duties provided in the basic contract.
- Work closely with clinical staff in developing patient wellness programs such as smoking cessation programs and asthma classes.
- Provide clinical guidance of corpsmen and pharmacy technicians.
- Provide medication information to the clinical staff as required, including in-service training on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head.
- Provide input to Clinical Pharmacist for Execute drug utilization reviews (DURs), as deemed necessary.
- Utilize pharmacy computer and robotic systems in accordance with MTF protocols.
- HCWs must perform inpatient and outpatient pharmacy functions.
- HCW must have understanding of the Tricare Uniform Formulary Tool, formulary and non-formulary medication screening, clinical interventions with healthcare providers, appropriateness of therapy, involvement in promoting pharmacy related events such as Patient Safety Month, medication adverse event reporting and tracking, technician education and guidance, patient counseling, educational presentations to staff, prescription transfers, medication reconciliation, and clinical pharmacy duties if supplemental privileges have been granted.
- Review hospital corpsman compounded pharmaceuticals (IVs, piggybacks) for appropriate concentrations, diluents, and drug interactions.
- Make evidence-based clinical recommendations to providers using a courteous, proactive approach to effective patient centered care.
- Provide, assess, and review information regarding the general administration of medications, toxicity, interactions, investigational drugs, and bioavailability. Monitor and review drug therapy for rational indications, dosing, adverse drug reactions, IV incompatibilities, and complications of multiple drug therapies in all patient care settings.
- The HCWs shall be responsible for the quality and timeliness of records, encounter closures within 72 hours, reports, and documentation of services provided. Document and communicate all information as appropriate including pain patient agreements, pharmacotherapeutic treatment findings and recommendations.
- Obtain a complete patient medication history, monitor the patient's response to drug therapy, educate and counsel the patient regarding his/her drug therapy. Counsel patients with special emphasis on therapeutic expectations, potential side effects, and any special education required for thorough understanding of the disease state.
- Design and conduct interdisciplinary educational programs in the areas of drug therapy for physicians, nurses and other health professionals. Provide educational and training programs to staff pharmacists, pharmacy intern students and pharmacy extern students.
- Collaborate on preceptorship duties with the Pharmacy Clinical Coordinator for pharmacy residents/students assigned to the Pharmacy Department.
- Monitor drug interactions and reporting of adverse drug events (ADEs).
- Ensure that work areas are clean and safe according to applicable regulations.
- Document significant medication interactions, and pharmacy interactions with prescribers.
- Renew medications for patients in accordance with policies and procedures approved by the DHA and MTF Pharmacy & Therapeutics committee.
- Customer Service Accountability - Contract employees providing services at NH Jacksonville and supporting facilities shall adhere to the Customer Service standards specified in NAVHOSPJAXINST 6320.7H or updated versions. A copy of the instruction will be made available upon request or at the time of commencement of services.
Minimum Qualifications:
- Bachelor's degree in pharmacy.
- Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
- Possess a minimum of 12-months of experience within the preceding 36-months as a pharmacist.
- Current, full, active, and unrestricted license to practice as a pharmacist.
- Must be a US Citizen.
