About this position as a Pharmacist
Reporting to the Chief Clinical Officer with close collaboration with CMO, VP Med Staff and IPU Director, the Clinical Staff Pharmacist provides clinical support for patient care as it relates to symptom management, clinical intervention, and medication management. The Clinical Staff Pharmacist will work directly with the medical and clinical teams to ensure high quality, cost-efficient pharmaceutical care. The Clinical Staff Pharmacist is expected to promote optimal patient care outcomes through the use of safe, cost-effective drug therapy and serve as a liaison to pharmacies for the organization.
This is a full time day position that is eligible for benefits.

What will you do as a Pharmacist at Hopehealth?
- Facilitates the identification of medication use issues including allergies, drug-drug interactions and duplication of therapy and collaborates with the clinical staff in optimizing patient care and therapeutic drug management.
- Provides pharmacy consultations to staff to optimize drug therapies in areas such as common hospice symptom management within the formulary (cost effective drug therapy, use of evidence-based medicine, palliative care guidelines, palliative sedation, and others that fall into hospice and palliative medicine).
- Works actively with hospice teams/IDGs to optimize patient-centered medication therapy and provide periodic and ad hoc education to hospice teams
- Provides education and documentation to team members to resolve and prevent drug-related issues.
- Review pharmacotherapy and facilitate discussion with patients, caregivers, and providers to reset therapeutic goals especially around ineffective, futile, and nonessential medications. Provides the necessary tools to providers for de-prescribing.
- Serve as a resource for staff on the optimal use of medications in hospice and palliative care.
- Monitors utilization patterns including formulary adherence, compounded medications, same-day delivery, and STAT delivery and provides recommendations for improvements that are patient centered and cost effective
- Participates in CII prescription reconciliation process as required by law.
- Assists in managing the Automated Dispensing Units medication management system for IPUs.
- Oversees the inpatient license renewals consistent with RI BOP and DEA regulations.
- Establishes and Chairs Pharmacy and Therapeutics Committee that will oversee safe and effective drug utilization and practice agency-wide.
- Works with Hospice Prescribers and Patient Care Managers to keep formulary updated and to review /approve all off-formulary requests..
- Works to mitigate drug shortages leveraging external relationships, provides recommendations for substitute/alternative drugs (route and dosing)
- Support the development and updating of medication-use algorithms that follow evidence-based best practices.
- Supports adherence of policies and procedures, which align with state and federal regulations including DEA, Massachusetts Board of Pharmacy, RI Board of Pharmacy, Joint Commission and Centers for Medicare and Medicaid Services (Medicare Conditions of Participation).
- Contributes to effective relationships with customers and vendors by serving as a liaison for local community pharmacies, contracted infusion pharmacies, and hospital partners.
- Participates on appropriate internal hospice committees.
- Performs other assignments, projects, and duties as assigned and/ or required.
- Participates on the process steering committee as pharmacy expert and assists in developing pharmacy -related SOPs.
What are the qualifications to be a Pharmacist at Hopehealth?
- Pharm D required.
- Licensed and Registered Pharmacist in MA and RI
- Two years clinical practice experience in pharmaceutical care for patients with acute, life-threatening illness, advanced illness, long term care or terminal illness (preference given to those candidates with hospice and/or palliative care experience) or completion of PGY-1/PGY-2 residency and/or fellowship program.
- Experience with pharmacy dispensing systems, electronic medical record documentation and use of common computer programs Microsoft Office suite. Outlook, Word, Excel, Power Point, etc.
- Experience with PBMs preferred
- Understanding of hospice regulations, policies and procedures preferred.
- Excellent verbal and written communication skills with individuals and groups.
