The Washington State Department of Corrections (DOC) is seeking a highly motivated and qualified professional to join the Health Services team as a Clinical Pharmacist, located at the Washington State Penitentiary (WSP) in Walla Walla, WA.
The Clinical Pharmacist supports the agency's mission by enhancing patient health and wellness via evidence-based pharmaceutical care directly to DOC patients receiving clinical services in DOC facilities and in preparation for reentry into the community. Retirement and Deferred Compensation

This position is directly responsible for managing pharmaceutical resources in accordance with DOC Pharmacy and Therapeutics (P&T) Committee direction, DOC Health Services Policy, and Federal & State Law.

Work as an active member of a multidisciplinary team and collaborates with healthcare providers:
- Maintain proficiency in DOC Health Plan provided services and knowledge necessary to provide safe and effective medication therapy, including evidence-based medication treatment, disease management, and legal and regulatory requirements related to pharmacy.
- Collaborate with the clinical team to individualize drug therapies promoting safe, effective, and economical patient care.
- Record/profile to identify, prevent or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication and treatment failures.
- Make appropriate evidence-based, patient-centered medication recommendations.
- Work under approved collaborative drug therapy agreements (CDTA) to initiate or adjust therapy to include the ordering and interpretation of laboratory testing.
- Participate in clinic/infirmary rounds and/or conferences with other healthcare professionals.
- Actively lead and/or participate in stewardship activities and medication monitoring/management programs such as anticoagulation management, pain management, diabetes care, oncology, antibiotic stewardship, etc.
- Serve as a pharmacist member of the Pharmacy and Therapeutics (P&T) Committee; attends all scheduled meetings.
- Review assigned Formulary sections as assigned for new safety, efficacy and/or cost information and presents to the P&T Committee.
- Develop drug monographs for P&T Committee review of new drug classes or new drugs for addition to the Formulary.
- Assist in the development of medication-specific protocols to guide treatment.
- Provide Health Care Entity oversight of assigned facility as Responsible Pharmacy Manager per WAC 246-945-310.
- Coordinate with central pharmacy, facility and statewide quality teams regarding medication therapy safety issues.
- Identify high-risk patients and implements interventions to improve quality and safety.
- Participate in the Medication Incident Review process.
- Actively attend local CQIP meetings.
- Provide education in-services to facility health services and other pharmacy staff.
- Present at educational conferences as requested.
