Job Description
Responsible for the coordination of activities required to assure that patients of the infusion pharmacy have a seamless safe transition home; with an appropriate plan of care for delivery of infusion therapy services in the home setting. Coordination of a safe transition will involve collaboration with multiple entities including but not limited to; the patient, patient's family and caregivers, infusion nursing team, central intake, payers, referral sources, physicians, distribution, and liaisons. Responsible to document the activities associated with the patient's transition to the home as well as progression to desired outcomes of the home infusion therapy in the organization's designated EMR.
Education And Credentials
Assocaites degree in nursing required. Registered Nurse licensure required. NYS Driver License required.
Experience
1 year of experience with infusion required
