Job Title: Physical Therapist

Job Specialty: Home Care - Urbana

Job Duration: 13 weeks

Shift: 8-hour days

Guaranteed Hours: 40 hours per week

Experience: Minimum of 1 year of experience preferred

License: Active Illinois PT License required

Certifications: CPR certification prior to patient contact

Must-Have: Own transportation with auto insurance; valid driver's license; excellent communication and interpersonal skills

Job Description:

- Responsible for visiting patients in their homes to perform therapeutic evaluations, exercises, and treatments to restore function and prevent disability following illness or injury, in accordance with state practice acts and home care rules and regulations.
- Demonstrate knowledge and skills necessary to provide age-appropriate patient care, including understanding the physical, emotional, and psychosocial needs of the patient population served.
- Evaluate patients' physical and functional status, including musculoskeletal and neurological systems, and functional mobility skills in activities of daily living.
- Analyze evaluation results and set appropriate short and long-term goals to support and develop the patients' plan of care.
- Select, administer, or monitor treatments, which may include modalities, therapeutic exercises, transfers, gait training, functional mobility training, and home safety.
- Determine appropriate assistive devices such as transfer aids, ambulation aids, orthotics, and prosthetics needed for functional mobility skills, including patient and family teaching as needed.
- Reassess patients' response to treatment and adjust programs accordingly, including ongoing communication with the physician, physical therapist assistants, patients or families, and other healthcare disciplines.
- Recommend other disciplines, programs, and equipment or supplies as needed by the patient or family.
- Evaluate the effects of physical therapy treatments and update the plan of care as needed.
- Maintain accurate and timely documentation in electronic medical records.
- Determine, instruct, and issue appropriate home programs.
- Adhere to universal precautions and infection control policies.
- Attend to the physical, emotional, and psychosocial needs of the patient population served.
- Attend patient care conferences, in-service education classes, and meetings as required.
- Stay informed of developments in the field of physical therapy and related disciplines to maintain and enhance professional expertise.
- Develop and maintain a positive working relationship with department and other home care personnel, patients, students, and outside agencies.
- Participate in continuous quality improvement efforts designed to improve patient outcomes and patient satisfaction efficiently.
- Work schedule may include some weekends and holidays as needed.
- Must be able to work independently in a virtual work environment and execute own workload.
