Job Description:
- Supervise and coordinate clinical rehabilitation activities for home health patients.
- Provide direct physical therapy services to patients in their homes.
- Collaborate with a multidisciplinary team to develop comprehensive care plans.
- Ensure compliance with regulatory requirements and best practices in home health.
- Conduct assessments and evaluations to establish individualized treatment plans.
- Provide guidance and support to therapy staff and ensure high-quality patient care.
- Participate in staff training and development programs.
- Maintain accurate documentation and records of patient care activities.
- Communicate effectively with patients, families, and healthcare professionals.
