Follows District rules and regulations maintaining appropriate licensure, certification, and authorization required for employment.

Follows all safety, fire and disaster regulations. Is knowledgeable about hazardous materials and Material Safety Data Sheets (MSDS) for chemicals used in the work area.
Reports safety hazards and injuries immediately to supervisor.

Assumes responsibility for a clear and orderly work area.
Dresses appropriately for work assignment as per dress code.

The employee makes good use of time and resources, and completes works assignments.
Appropriately and consistently uses the time keeping system and completes corrections in a timely manner.
Performs work in a cost efficient method. Uses District resources appropriately.
Meets or exceeds District rules on attendance and tardiness.

Shift: Days
Hours: 08:30am - 5:00pm
Schedule: Monday thru Friday
Primary Duties
1. Maintain established departmental policies and procedures, objectives, qualify improvement program, safety, environmental and infection control standards.
2. Evaluate referred patients and develop treatment plans including specific goals and activities, reevaluating and modifying plans as necessary.
3. Provide instructions in therapeutic procedures to be continued by the patient.
4. Provide direct physical therapy to patients per plan of care.
5. Maintain active and support communication with the patient/caregiver regarding progress, problems, home programs and other issues related to the therapeutic process.
6. Prepare written documentation as required by the profession and the department such as evaluation results, individualized treatment plans and progress reports.
7. Communicate observations, facts and comments with the interdisciplinary team.
8. Attend patient care conferences and other related meetings.
9. Assist in the Quality Improvement Program and in providing inservices to other staff.
10. Enhance professional growth and development through participation in education programs, current literature, workshops and inservice meetings.
11. Demonstrate sensitivity to the needs of all cultural and age groups, predominantly the geriatric and dying patient.
Position Qualifications
Minimum Education
Bachelor's Degree in Physical Therapy
Minimum Experience
2 year's experience in physical therapy
Required Certification/Registration
CPR Certification
California Physical Therapy License
Valid CA Driver's license
Continuous automobile insurance coverage
Preferred Courses/Training
Home Health Experience
