Job Description:
- Provide home health physical therapy services.
- Develop and implement individualized therapy plans for patients.
- Assess and document patient progress toward meeting desired outcomes.
- Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.
- Educate patients and families on therapeutic exercises and activities.
- Maintain patient confidentiality in accordance with regulations and policies.
- Utilize clinical expertise to improve patient mobility and function.
- Adapt treatment plans as necessary to meet the individual needs of each patient.
- Ensure high-quality care while meeting productivity expectations.
