Experience: Minimum of 3 years
License: Active Colorado Physical Therapy License or compact license
Certifications: Current CPR (BLS) certification (within 90 days of hire)
Must-Have: Valid Driver's License (for Home Health only)
Job Description:
- Deliver full-time outpatient physical therapy with a focus on patient care, team collaboration, and a growth mindset.
- Provide 45-minute one-on-one treatments and 1-hour evaluations with paid documentation time.
- Work in a rural clinic treating a variety of conditions including pediatrics, geriatrics, orthopedics, post-op, sports medicine, vestibular/concussion, and long-term neurological conditions.
- Collaborate within a supportive, multidisciplinary team environment, with opportunities for mentorship and continuing education.
- Assess and interpret evaluations and test results to determine physical therapy treatment plans.
- Diagnose and manage movement dysfunction, enhancing physical and functional abilities.
- Restore, maintain, and promote optimal physical function, wellness, fitness, and quality of life related to movement and health.
- Prevent the onset and progression of impairments, functional limitations, and disabilities.
- Recommend assistive devices and provide gait training as necessary.
- Conduct discharge planning in consultation with interdisciplinary teams.
- Document patient care services using EMR charting, adhering to industry standards.
- Uphold a safe and clean working environment by complying with procedures, rules, and regulations.
- Protect patients and employees by adhering to infection-control policies and protocols.
- Maintain professional and technical knowledge through educational workshops and professional societies.
- Develop rehab and hospital staff by providing information and conducting in-service training programs.
- Adhere to legal and certification requirements and advise management on needed actions.
- Encourage health, create a safe treatment environment, and motivate patients with a good bedside manner and quality focus.
- Participate in ongoing professional development to maintain best practice standards.
- Critically assess self to improve clinical and professional performance and advocate for the physical therapy profession.
