We are seeking a dedicated and compassionate Physical Therapist or Occupational Therapists to join our team for a home care role. The ideal candidate will have a passion for patient care and the ability to work independently, driving from patient to patient to provide physical therapy services in the comfort of their homes.

Key Responsibilities:

• Patient Care: Provide high-quality physical therapy services, primarily focusing on routine visits with some assessments.
• Assessment and Planning: Conduct thorough evaluations to determine patient needs and create personalized care plans.
• Therapeutic Intervention: Implement and monitor therapy programs designed to improve mobility, reduce pain, and restore physical function.
• Documentation: Maintain accurate records of patient progress, treatment plans, and outcomes.
• Communication: Collaborate with healthcare providers, family members, and caregivers to ensure coordinated care.
• Patient Education: Educate patients and their families on exercises and activities to enhance therapy outcomes and promote long-term well-being.
• Travel: Travel between patients' homes within the designated area, adhering to a schedule that meets both patient and company needs.

Qualifications:

• Education: Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Licensure: Valid state licensure as a Physical Therapist.
• Experience: Previous experience in home care or a similar setting is preferred but not required.
• Skills: Strong clinical skills, excellent communication, and the ability to work independently.
• Transportation: Must have a reliable vehicle, a valid driver's license, and current auto insurance.
