Key Responsibilities:
- Patient Care: Provide high-quality physical therapy services, primarily focusing on routine visits with some assessments.
- Assessment and Planning: Conduct thorough evaluations to determine patient needs and create personalized care plans.
- Therapeutic Intervention: Implement and monitor therapy programs designed to improve mobility, reduce pain, and restore physical function.
- Documentation: Maintain accurate records of patient progress, treatment plans, and outcomes.
- Communication: Collaborate with healthcare providers, family members, and caregivers to ensure coordinated care.
- Patient Education: Educate patients and their families on exercises and activities to enhance therapy outcomes and promote long-term well-being.
- Travel: Travel between patients' homes within the designated area, adhering to a schedule that meets both patient and company needs.
Qualifications:
- Education: Bachelor's or Master's degree in Physical Therapy from an accredited program.
- Licensure: Valid state licensure as a Physical Therapist.
- Experience: Previous experience in home care or a similar setting is preferred but not required.
- Skills: Strong clinical skills, excellent communication, and the ability to work independently.
- Transportation: Must have a reliable vehicle, a valid driver's license, and current auto insurance.
