Physician - Cardiology

OUTPATIENT RHEUMATOLOGY PHYSICIAN SERVICES (ON-SITE SPECIALTY CLINIC SERVICES & TELEHEALTH SERVICES) Will be flexible to allow for short term (13 weeks), long term (1-year to 3-year) and, where feasible, include intermittent support to meet unplanned needs that develop on a frequent basis. RHEUMATOLOGY PHYSICIAN SERVICES (ON-SITE SPECIALTY CLINIC SERVICES & TELEHEALTH SERVICES) The period of performance (or delivery) is Base Period 04/01/2025-03/31/2026 First Option Period 04/01/2026-03/31/2027 Second Option Period 04/01/2027-03/31/2028 Chinle needs a contractor to provide On-Site Specialty Clinic Services & Telehealth Rheumatology Physician Services. Services shall be provided in accordance to attached Performance Work Statement. Rheumatology Duties: 5.2.1 Work under the general technical, administrative supervision of Donald Miles, MD, CCHCF Acting Primary Care Director or her designee. 5.2.2 The contractor shall provide professional medical services or telemedicine services under the terms of this contract, appropriately and timely medical services in accordance with the standards of care established by recognized medical care organizations and in accordance with the policies and procedures of Service Unit's Medical Staff Bylaws and Rules & Regulations at CCHCF. -Duties and responsibilities: will encompass outpatient, Rheumatology Physician Services (On Site Specialty Clinic Services and Telehealth Services) to I.H.S. patients. -Place of Performance: The patient shall be at CCHCF Outpatient Department, Specialty Clinic. The contractor shall be allowed to remote in and perform the visit with the patient via IHS sanctioned telecommunications platform. The contractor may chart into the EHR system remotely. Telemedicine Services: 5.1.1. Telemedicine: The contractor shall provide telemedicine services in addition to on-site clinics. These services will be provided with the patient on-site at CCHCF via a secure audiovisual connection on the IHS network. The contractor shall document these encounters in the IHS EHR. These services will be billable only for regularly scheduled telemedicine specialty clinics. 5.1.2 Telephone: The contractors shall offer telephone visits to patients when an audiovisual connection with patient is not possible/feasible
OUTPATIENT RHEUMATOLOGY PHYSICIAN SERVICES (ON-SITE SPECIALTY CLINIC SERVICES & TELEHEALTH SERVICES) Telehealth: Healthcare services delivered via telemedicine, or asynchronous (store and forward). 2.32 Telemedicine: The practice of health care delivery, diagnosis, consultation, and treatment and the transfer of medical data through interactive audio, video or data communications that occur in the physical presence of the patient, including audio or video communications sent to a health care provider for diagnostic or treatment consultation. Telemedicine includes: Synchronous (real-time): Two-way interaction between a person (patient, caregiver, or provider) and a provider using interactive audio and video. Remote patient monitoring: Personal health and medical data collection from an individual in one location via electronic communication technologies, which is transmitted to a provider (sometimes via a data processing service) in a different location for use in providing improved chronic disease management care and related support.
