Job Description
The Medical Director, Population Health will be the physician primarily responsible for driving improvement related to value based care. As Valley Health continues to pursue opportunities in both pay for performance in value based care initiatives, the organization requires a physician liter to focus on improvements in these areas. The medical director will be an experienced physician leader with a demonstrated record of improvement in quality of care.
Specific responsibilities will include:
Will function as the primary liaison to the primary care providers in VHMG to facilitate value based clinical transformation.
Review of monthly data from payers and internal sources describing opportunities in value based care and pay for performance initiatives.
Examination of these data sets for opportunity and stratification to inform work.
Design of communication and action strategies to encourage other providers to improve the quality of care in these areas.
Review of team based interventions designed to improve outcomes.
Collaboration in the development of nurse navigation and chronic care management programs to improve the care of chronically ill patient's between office visits and improve outcomes.
Participation in the Quality committee and other VHS committees as needed.
Represent VHS on MG and CIN related Quality Committees.
Maintain an active and successful clinical practice.
Education
MD/DO with BC Required
Primary Care specialization preferred
Experience
Five years or greater of primary care practice experience required
Physician leadership experience in population health and/or quality preferred
Certification & Licensures
Board certified in Family Medicine, Pediatrics, or Internal Medicine required
Qualifications
Must have the ability to work well with interdisciplinary team of healthcare providers
Must possess and maintain an unrestricted license to practice in the Commonwealth of Virginia or be eligible to obtain
Must have professional liability insurance coverage in such amounts and at such rates acceptable to VPE
Must have an unrestricted valid DEA number, to dispense controlled substances
Must have participating provider status with Medicare, Medicaid, and other health plans or payors with which VPE has contracts
Competent to provide assessment, care and treatment appropriate to the age group of patients served
