Key Responsibilities:

• Provides comprehensive primary care services to a diverse patient population, including LGBTQ+ individuals.
• Develops and implement individualized care plans, emphasizing preventive care and patient education.
• Collaborates with multidisciplinary teams to address the physical, mental, and social health needs of patients.
• Participates in community outreach programs to promote health and wellness.
• Engages in ongoing professional development and contribute to clinical research initiatives.
• Supports culture enhancement efforts and serves as a role model for performance excellence.

Qualifications:

• Medical degree (MD or DO) with board certification in Family Medicine, Internal Medicine, or a related field.
• Unrestricted license to practice medicine in the state of Illinois.
• Demonstrated commitment to serving LGBTQ+ communities and marginalized populations.
• Strong interpersonal and communication skills, with the ability to work effectively in a team-based setting.
• Experience in a community health setting is preferred but not required.
