Position Overview:
We are seeking a dedicated Physician - Adult Medicine to join our dynamic team. The ideal candidate will provide comprehensive primary care services to adult patients, promoting health and wellness while managing acute and chronic medical conditions.
Key Responsibilities:
Provide high-quality, patient-centered medical care to adult patients.
Diagnose, treat, and manage acute and chronic medical conditions.
Develop individualized care plans in collaboration with patients and interdisciplinary care teams.
Educate patients on disease prevention, health maintenance, and treatment options.
Participate in quality improvement initiatives and care coordination efforts.
Document all patient interactions accurately and promptly in the electronic health record (EHR) system.
Work collaboratively with medical, behavioral health, dental, and pharmacy teams to provide integrated care.
Participate in on-call rotation as needed.
Requirements
Qualifications:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
Board-certified or board-eligible in Internal Medicine, Family Medicine, or a related field.
Valid and unrestricted medical license to practice in Kentucky.
DEA license.
Experience working in a primary care or community health setting preferred.
Commitment to serving underserved populations and promoting health equity.
Strong interpersonal and communication skills.
Ability to work effectively as part of a multidisciplinary team.
