Position Summary
The Population Health Manager of Veterans Health Initiatives provides service and administrative oversight to the Veterans Health Initiative (VHI) team within our Certified Community Behavioral Health Clinic (CCBHC) / Federally Qualified Health Center (FQHC). This role ensures the delivery of high-quality, culturally competent, and integrated services to veterans and their families experiencing mental health, substance use, and co-occurring disorders. The Manager will be responsible for peer specialist oversight, program development, collaboration with interdepartmental and community partners, and the use of health data to track trends, risks, and outcomes to enhance access to care and improve health outcomes for veterans.

Physical Demands and Work Environment
The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands and arms to reach. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.
The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.

The Manager will be responsible for peer specialist oversight, program development, collaboration with interdepartmental and community partners, and the use of health data to track trends, risks, and outcomes to enhance access to care and improve health outcomes for veterans.
Required Qualifications
- Bachelor's degree in healthcare administration, social work, counseling, psychology, or a related field (Master's degree preferred).
- Minimum of 3 to 5 years of experience in behavioral health or veteran's services.
- Knowledge of veteran-specific issues, including military culture, trauma, and transition challenges.
- Demonstrated experience in program development, implementation, and evaluation.
- Strong leadership, supervisory, and organizational skills.
- Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
- Excellent communication and interpersonal skills.
- Assist and support the Center/Department/Program to meet standards of High Reliability.
- Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking, and social distancing.
- Knowledge of trauma-informed care and evidence-based practices for PTSD and SUD.
- Familiarity with CCBHC/FQHC models of care is a plus.
- Experience with electronic health records is a plus.
- Military service experience is a plus (veterans strongly encouraged to apply).
Essential Responsibilities
Program Development and Implementation:
- Develop, implement, and evaluate programs and services tailored to the unique needs of veterans and their families.
- Identify and address gaps in services and develop strategies to improve access and quality of care.
- Stay abreast of current best practices and evidence-based interventions for veterans with mental health and substance use disorders.
- Assist in the collection of data and the creation of reports related to the veteran health initiative (VHI).
- Ensure adherence to organizational policies, procedures, and ethical standards.
Services and Care Coordination:
- Provide direct services, as needed, including assessments, individual and group psychoeducation, and crisis intervention.
- Oversee the development and implementation of individualized care plans for veterans, ensuring integration of behavioral health, primary care, and other support services.
- Facilitate care coordination with VA healthcare systems, community-based organizations, and other providers.
- Ensure appropriate documentation and compliance with all regulatory requirements.
Community Collaboration and Advocacy:
- Build and maintain strong relationships with community partners, including veteran service organizations, VA medical centers, interdepartmental leadership, and other relevant agencies.
- Advocate for the needs of veterans and their families within the community and at the state and national levels.
- Participate in community outreach and education activities to raise awareness about veterans' mental health and substance use issues.
Quality Improvement and Compliance:
- Use health data to monitor and evaluate program outcomes and implement quality improvement initiatives.
- Ensure compliance with CCBHC/FQHC standards, licensing requirements, and funding regulations.
- Participate in audits and reviews as required.
- Maintain current knowledge of all relevant HIPAA and 42 CFR Part 2 rules.
