Overview

The Nurse Practitioner Cardiogenic Shock Program Manager is responsible for providing leadership, expertise, collaboration, consultation and mentorship for the team responsible in promoting evidence based practice in the care of the Cardiogenic Shock patient. The Manager is an expert and patient advocate in the management of patients requiring advanced cardiogenic shock therapies including percutaneous mechanical circulatory support. This position manages a group of clinically complex or resource intensive patients and aligns with evidence based medical benchmarks (SCAI Shock Initiative, AHA, ACC, STS, i.e.). The Cardiogenic Shock Manager is the Program Leader in a hub and spoke model supporting enterprise wide strategic and organizational goals under the guidance and direction of the Vice President and Medical leadership of the Cardiovascular Care Transformation Services (CTS) and Shock Program Medical Director in conjunction with campus and program Medical Director(s), Physician(s)/ Campus specific Administration(s) and various CV leaders to achieve optimal standards of care with measurable identified outcomes. Minimum 6 years of experience as an APN. 4. Minimum 5+ Years of Experience in cardiovascular surgery and interventional cardiology.

Responsibilities

1. Works in conjunction with the VP Cardiovascular Care Transformation Services (CTS) and site specific leadership teams to develop programs that achieve accreditation requirements, quality care delivery, and desired patient experience. Develops position descriptions for each area, defines work processes/protocols, and develops/implements consistent performance standards for each area across the HMH CV CTS Network. In collaboration with CTS and regional campuses assists in development of the strategic planning initiatives for cardiogenic shock and related programs (Advanced HF (VAD)) across the Network in collaboration and standardization enterprise wide with the advanced HF team. 2. Responsible for program data management including but not limited to: formal data tracking and submission for DSCs; development and maintenance of performance dashboards or scorecards to support management of the spine programs. Track, analyze, and report on relevant data and metrics as needed and/or requested. Develop, recommend and/or implement actions or process improvements as appropriate. 3. Part of the Leadership team responsible for the creation of the Cardiogenic Shock team initiative. Partners with Cardiology, Advanced Heart Failure, Cardiac Surgery and Critical Care teams in evaluating and treating with mechanical circulatory support for the Network. 4. Practices as an autonomous and collaborative member of the healthcare team, demonstrating and teaching clinical expertise, differentiating between normal findings and those needing treatment, referral and or consultation. 5. Lead process improvement efforts to address gaps and align with evidence based, standardized care for the cardiogenic shock population. 6. Demonstrates advanced assessment skills, within clinical specialty, and in the development of the cardiogenic shock team in providing evidence-based treatment interventions, to achieve positive outcomes for patients and families. 7. Coordinates people, places and products to provide clinical guidance and specialty leadership through participation and/or representation at unit, departmental and hospital meetings, committees, councils, task forces, health fairs, educational presentations and multidisciplinary rounds, as needed. 8. Develops educational resource materials independently and in collaboration with other health care providers. Functions as a mentor, educational resource, consultant, for staff providing clinical expertise. Serves as a resource and support person for ventricular assist device patients and their family members both in the acute care setting and in the community. Works in conjunction with Nursing and Departmental Education to ensure clinical competency for all staff caring for cardiogenic shock patients. 9. Participates in the development and recommendation of evidence-based practice identifying the goal and desired measurable outcomes, which may include clinical, financial, process/system and patient/family satisfaction. 10. Explores opportunities to expand the existing scope of services to grow revenue and improve quality care delivery in alignment with the Cardiovascular Transformation Team `s strategic vision. Manages time and resources effectively by managing multiple priorities with appropriate delegation. Develops, implements, and supports activities and/or projects that promote patient satisfaction within the Office of Human Experience framework to achieve best practice targets. 11. In collaboration with the CV Transformation leadership, assists in the development of cardiogenic shock protocols and its relation to our enterprise wide marketing campaigns and community outreach. 12. Educate all staff regarding care of cardiogenic shock patients in collaboration with the cardiovascular attending of record to prevent unnecessary readmissions. 13. Provide preceptorship to the other sites within the network. 14. Precept APP students in clinical practicum. 15. Participate at various national and local meetings and represent Hackensack Meridian Health on the topic of cardiogenic shock. 16. Functions as APN, as needed. 17. Other duties and/or projects as assigned. 18. Adheres to HMH Organizational competencies and standards of behavior. 19. Lifts a minimum of 25 lbs., pushes and pulls a minimum of 25 lbs. and stands a minimum of 6 hours a day. Requires ability to travel to HMH Tertiary care hospitals, physician offices, and administrative locations through the network. Requires time to support program events after hours and weekends as needed to promote the programs.

Qualifications

Education, Knowledge, Skills and Abilities Required: 1. Graduate of NLN/AACN accredited program in nursing. 2. Master's Degree. 3. Minimum 6 years of experience as an APN. 4. Minimum 5+ Years of Experience in cardiovascular surgery and interventional cardiology. 5. Excellent written and verbal communication skills. 6. Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms. Education, Knowledge, Skills and Abilities Preferred: 1. Shock experience. Licenses and Certifications Required: 1. Advance Practice Nurse License. 2. NJ State Professional Registered Nurse License. 3. BLS AHA Basic Health Care Life Support HCP Certification. 4. Advanced Cardiac Life Support Certification. Licenses and Certifications Preferred: 1. Not Applicable
