About the Role:

Unity Health Care is seeking a skilled Chronic Care and Off-Site Clinics Database Manager to oversee the development and use of health care databases at Corp-Unity Health Care Inc. at DOC that will play a pivotal role in enhancing the quality of care for patients with chronic conditions. This position is responsible for developing, implementing, and overseeing programs that aim to improve patient outcomes and ensure efficient resource utilization. The Program Manager will collaborate with healthcare professionals, patients, and stakeholders to create tailored care plans that address the unique needs of individuals with chronic illnesses. By analyzing data and feedback, the Program Manager will continuously refine programs to meet evolving healthcare standards and patient expectations. The Manager's responsibilities include managing and maintaining databases while ensuring high levels of data availability for medical staff. The candidate will also be responsible for developing, testing, improving, and maintaining new and existing databases to help users retrieve data effectively. As our ideal candidate, you have proven experience as a Chronic Care and Off-Site Clinics Database Manager, and you are well-versed in data administration and management process. Ultimately, this role is crucial in driving initiatives that promote better health management and improve the overall patient experience within the chronic care spectrum.

Minimum Qualifications:
  • Bachelor's degree in Nursing, Public Health, Healthcare Administration, or a related field.
  • Strong understanding of chronic diseases and their management.

Preferred Qualifications:
  • Master's degree in a relevant field.
  • Experience with data analysis and program evaluation.
  • Certification in Chronic Care Management or related specialty.

Responsibilities:
  • Develop and implement chronic care management programs that align with organizational goals.
  • Collaborate with healthcare providers to create individualized care plans for patients with chronic conditions.
  • Monitor program performance through data analysis and patient feedback to identify areas for improvement.
  • Facilitate training and support for staff involved in chronic care management.
  • Engage with patients and their families to educate them about chronic disease management and available resources.

    Responsibilities:
    • Collection and management of chronic care clinic (CCC) enrollment data from intake stratified by acuity levels.
    • Monitor CCC assignments, schedules and follow up with the chronic care providers team so chronic cares services are delivered and follow ups are monitored without interruption or delays.
    • Assuring patients are registered in all Chronic Care correctly to match problem list.
    • Monitor creation of schedules in EMR as well as printed Schedules to Medical records, officers and providers for chronic care clinic sessions.
    • Manage and monitor scheduling in EMR- includes movement of schedules, creation of appointments and cancellation of appointments.
    • Weekly reports consist of Weekly Data Sheet and Weekly schedules with clarification of missed appointments, transfers, releases, rescheduled refusals, offsite appointment follow ups. Reports are emailed to Data Collector, RMD, Med. Director, HSA, DON and ADON.
    • Maintains and monitors off site schedules with development of a dashboard for scheduling, completion of the consults and follow ups with their CCC providers team to close the consultation loop.
