POSITION SUMMARY:
Community Healthcare Network is seeking a Program Manager/Ryan White. The Program Manager is responsible for the coordination and supervision of the Ryan White Part B HIV/AIDS Support Services that address Social Determinants of Health (SDOH) for patients receiving care at the Brooklyn Clinics. This program provides case management and support services to HIV-positive clients with emphasis in addressing the SDOH as dictated by the grant award enhancing clinical outcomes, maintaining patient retention, and facilitating support services and access to care. The Program Manager is responsible for ensuring that all program deliverables are met including data collection, timely monthly reporting to the AIDS Institute, and reporting to the VP of ID/LGBTQ Programs and the Board of Directors as needed. In conjunction with the Director of ID/LGBTQ Programs, the Program Manager will develop, implement, and update an appropriate QI plan to ensure proper monitoring of program deliverables. This plan should be presented to CHN's Quality Advisory Committee. The Program Manager will establish appropriate links (via Memorandums of Understanding) with other communities that could supplement services not directly provided by CHN.
DUTIES AND RESPONSIBILITIES:
Works directly with the VP, ID/LGBTQ Programs to create and maintain a work environment where mutual support and regulatory compliance is fostered. Works with the Director of ID/LGBTQ Programs to develop a timely data input that follows the AIRS mapping and monthly reports Assigns team responsibilities, addresses conflicts, and provides support to ensure a productive and collegial work environment. Works with program staff and clinic staff to solve problems, improve processes, and ensure quality. Provides bi-weekly supervision for the Supportive Services Case Management staff. Develops client-focused service plans with documented input and approval from the clients. Coordinates client services with internal and external service providers through regular case conferencing and advocacy. Facilitates the provision of empowerment support and health education groups. Ensures that Supportive Services Case Managers plan and execute all case management sessions, outreach activities, health education groups and psychosocial support groups in a timely and appropriate manner. Ensures a caseload of 75 clients per year per case manager is met. Coordinates with CHN Health Center Directors to ensure that the program is in compliance in regards to establishing appropriate linkage agreements with designated medical and social service agencies. Establishes a system for follow-up on referrals to other support services not provided for under this contract. Develops standard policies and procedures. Participates in Quality Improvement activities as required. Conducts monthly chart reviews to ensure regulatory compliance. Effectively organizes multiple assignments, sometimes of complex nature or involving completing priorities, to produce documentation that is accurate thoroughly and on time. Attends all HIV Team meetings to verify client medical status and engage in group case conferencing. Attends all grants management meetings, CHN staff meetings, and HIV Care Network Meetings. Coordinates mandated training opportunities and staff development within required timeframes. Performs other related duties as assigned by supervisor.
EDUCATION/EXPERIENCE required:
Bachelor's Degree or satisfactory equivalent of education and work experience required Minimum of three (3) years of administrative experience with demonstrated supervisory or program oversight experience required Familiarity with HIV care services, including but not limited to, Anti-Retro-viral therapies (ARV's), Adherence, and viral suppression
