WHO WE ARE LOOKING FOR
The Coordination of Community Services Program within the Community Support Network in Aging and Disability Services is currently recruiting for an enthusiastic professional who is dedicated to providing high-quality case management services to individuals with intellectual and developmental disabilities. The incumbent will serve as a Coordinator and will facilitate Person-Centered Plan development, coordinate resources and support and monitor service implementation to assist individuals who have been determined eligible by the Developmental Disabilities Administration (DDA) with obtaining their best life.
What You'll Be Doing
The Program Specialist II provides targeted case management services for adults and children with developmental disabilities. Duties include, but are not limited to:
The position requires travel using a personal vehicle to perform field visits to coordinate and monitor services.
A background investigation will be conducted on the selected candidate prior to appointment and will be a significant factor in the hiring decision.
Minimum Qualifications
Experience: Two (2) years of professional experience working with adults and children with developmental disabilities.
Education: Graduation from an accredited college or university with a Bachelor's Degree. (NOTE: For positions in the DHHS' Coordination of Community Services Program: Graduation from an accredited college or university with a Bachelor's Degree in a human services field. No substitution is allowed.)
Equivalency: An equivalent combination of education and experience may be substituted. (Note: For positions in the DHHS' Coordination of Community Services Program, education and experience requirements may be waived if an individual has been employed as a coordinator of community services for at least 1 year as of January 1, 2014)
License: Possession and maintenance at all times of a valid Class C (or equivalent) driver's license from the applicant's state of residence when required for job-related duties

- Meeting with private/public providers. Coordinating with agencies such as child welfare, adult protective services.
- Performing other job-related duties as assigned.
