Purpose:

The purpose of this position is responsible for ensuring members served by our Collaborative of Safety Net Hospitals and Community Mental Health Organizations are provided psychiatric services upon discharge from hospital based care and through enrollment in Collaborative Bridges partner services. This member of the multidisciplinary team is responsible for conducting assessments, assessing physical health needs, making and supporting appropriate referrals to community care, and prescribing and administration of medications. This member should have full psychiatric prescriber capabilities. To ensure coordinated transitions of care, they will work with hospital staff to ensure understanding of recommended treatment plan. This professional will provide psychiatric consultation to staff and administrators and works as an integral part of the clinical, multidisciplinary team on Collaborative Bridges who provide integrated crisis stabilization care to high acute behavioral health populations. In addition, this professional will provide support, as needed, on site at our shared Collaborative Bridges Wellness Center in West Garfield Park in supporting individuals in need of ongoing psychiatric supports enrolled in intensive team based program, such as Community Support Team and Assertive Community Treatment models of care.

Collaborative Bridges is a partnership of historic West Side Chicago safety net hospitals (Humboldt Park Health, Loretto Hospital, and Hartgrove Hospital) and community mental health agencies (Habilitative Systems, Inc, Bobby E Wright, and Community Counseling Centers of Chicago) designed to ensure that the care of people experiencing mental health and substance use needs are supported in their community, especially after a hospital discharge. We are creating a level of integration and uninterrupted continuity of care between hospitals and community organizations to put resources closer to those who need it, when they need it most. Our multi disciplinary teams provide immediate crisis stabilization therapy services for both substance use and mental health needs in addition to providing supports in addressing the underlying stressors and barriers to recovery related to health related social needs. We are closing the gaps among disjointed islands of service on Chicago's West Side, through a deeper level of integration of helping systems. Duties and Responsibilities

  • Provide psychiatric care, including evaluations, follow ups, and urgent and emergent consultations to individuals with mental health needs.
  • Facilitate outpatient psychiatric patient visits for the evaluation and management of established patients.
  • Ability to maintain strict client confidentiality.
  • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, and other pertinent information.
  • Recognize impact of Social Determinants of Health factor as drivers for behavioral health needs and implement intervention and evaluation approach.
  • Evaluate patients for the need for a higher level of care, including hospitalization
  • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare service.
  • Prescribe psychiatric medications as indicated.
  • Ability to be respectful of the diverse cultures of the people served and to provide culturally appropriate, competent, and individualized treatment according to each client's age, gender, race, ethnicity, and culture
  • Assist with ensuring hospital discharge recommendations are understood by consumer and followed through with by the patient. '
  • Ensure interventions are recovery-oriented, culturally congruent, and developmentally appropriate
  • Maintain high quality documentation of all case work, completed in a timely manner, and consistent with outlined program policies and licensure requirements.
  • Work collaboratively with interdisciplinary teams and health care team members both internal and external to the organization to improve patient care through effective utilization and monitoring of health care resources.
  • Other medical duties necessary for carrying out the position.

Minimum Qualifications:
  • RN License
  • Graduate degree in nursing or nursing practice
  • APN Certificate from an accredited body
  • License to prescribe controlled substances
