Job Description:

- Provide physical therapy services to patients in a skilled nursing facility setting.
- Collaborate with a team of healthcare professionals to develop and implement individualized patient care plans.
- Evaluate and document patient progress toward treatment goals.
- Adapt treatment plans as necessary to ensure optimal patient outcomes.
- Maintain a safe and clean working environment by complying with procedures, rules, and regulations.
- Participate in team meetings and training sessions as required.
- Educate patients and their families on home exercise programs and injury prevention.
