Summary
Pathways clinic at Downtown Public Health is hiring for a Public Health Nurse to work as part of our phenomenal team. Pathways, located in the Downtown Public Health building is an innovative program that uses a harm reduction approach to help address the opioid epidemic and provides low barrier access to Medication for Opioid Use Disorder (MOUD), primarily buprenorphine. This Public Health Nurse in the Pathways Clinic will form part of our new mobile team providing services in the field in the Seattle area. As part of an interdisciplinary team, this PHN will administer long-acting injectable medication for opioid use disorder (MOUD), provide medication counseling, and address concerns related to opioid and other substances use.

Job Duties
  • Works in partnership with interdisciplinary team as part of Pathways mobile team providing counseling and administering long-acting injectable medication for opioid use disorder (MOUD).
  • Provides care management, assessment, education, and close follow-up for patients receiving MOUD.
  • Coordinates safe storage & handling, transportation, administration, and documentation of medications for opioid use disorder.
  • Maintains partnership with other outreach programs (street medicine, Robert Clewis center Needle exchange program, etc.) to provide services to vulnerable patients in the community facing opioid and substance use.
  • Conducts medical screening and triage with patients on the field and provides appropriate follow up care.
  • Conducts nursing assessments and interventions, patient care planning, evaluation, and medical care coordination.
  • Provide point of care follow-up and preventive screenings for patients undergoing treatment for Opioid Use Disorder
  • Lead quality assurance projects for clinic workflow and coordination of care with other healthcare and social welfare entities.

Experience, Qualifications, Knowledge, Skills
Requirements:
  • Bachelor's degree in nursing science or a graduate level nursing degree of MSN, DNP or PhD from an accredited school of nursing.
  • Current Washington State license as a Registered Nurse or Multi-State Licensure.
  • Pass background, credentialing and pre-employment physical.
  • Documentation of current BLS-CPR for Health Care Providers. CPR/AED HeartSaver courses do not meet this requirement.
  • Washington state driver's license.

Work Location & Schedule: This position is based out of Downtown Public Health Center but will work in various locations. This is a 1.0 fte, fully benefited, career service position with a typical work schedule of Monday-Friday, 8 a.m. and 5 p.m. and is overtime eligible. May work outside of the typical schedule to meet operational needs.
Working Conditions: Work could be conducted in a variety of locations that could include indoors, in office, on a vehicle, and outdoors. Travel throughout King County to various locations where unsheltered people are living is required. Outreach work is conducted in uncontrolled outdoor environments and will require the employee to traverse over uneven surfaces and obstacles, ascend and descend slopes, and move through wooded areas without maintained trails while transporting gear weighing up to 35 lbs. The employee will frequently work in and around extreme heat and cold, precipitation, ambient smoke, loud noises, individuals who are distressed, intoxicated, or displaying unpleasant behavior, and potentially biohazardous material. Must constantly perceive surroundings for and communicate with team about potential hazards under these conditions. Appropriate training and PPE will be provided. Must be able to communicate clearly and quickly with team members and clients and be responsive to phone calls and messages throughout the day. Must remain calm in adverse conditions and respond appropriately to emergency situations as they arise following established safety protocols. Employee may be called upon to administer naloxone and / or provide basic life support care to individuals in emergency situations, alone or as a member of a team. Employee will be required to share responsibility for driving the outreach vehicle(s) up to 60 miles per day in all weather conditions. The employee will also be required to move boxes, supplies, and packs weighing up to 35 lbs. between ground level and 5ft to load and unload outreach supplies.
