Responsibilities:
Client Care and Assessment:
- Conduct nursing assessments to evaluate physical health, mental health, and substance use needs.
- Provide direct client care, including administering medications, monitoring vital signs, and addressing immediate health concerns.
- Perform wound care, injections, and other clinical procedures as necessary.
- Document and update client health records accurately and timely in compliance with CCBHC policies and regulations.
Collaboration and Support:
- Collaborate with interdisciplinary teams, including social workers, therapists, psychiatrists, and case managers, to create and implement client care plans.
- Attend case conferences and treatment planning meetings to provide insights into client health needs.
- Educate clients and their families on managing health conditions, medications, and treatment plans.
Medication Management:
- Administer prescribed medications, monitor for adverse effects, and document any reactions or side effects.
- Educate clients about prescribed medications, including proper use and potential side effects.
- Ensure medication adherence and help clients understand the importance of their treatment plan.
Crisis Intervention and Support:
- Provide immediate support in crisis situations, including offering physical health evaluations, coordinating emergency care, and de-escalating tense situations.
- Assist with referrals to community resources for additional behavioral health or medical needs.
Client Advocacy and Education:
- Act as an advocate for clients, ensuring their voice is heard in treatment decisions and helping them navigate the healthcare system.
- Provide education on wellness and preventive health strategies, especially in relation to mental health and substance use recovery.
Documentation & Compliance
- Maintain accurate and up-to-date client records, including assessments, care plans, and progress notes.
- Ensure compliance with New York State regulations and standards for nursing practice and behavioral health services.
- Prepare and submit required reports related to client care, staff performance, and clinical outcomes.
Collaboration & Communication
- Work closely with other healthcare professionals, including physicians, social workers, and therapists, to provide integrated care.
- Communicate effectively with clients, families, and staff, fostering a collaborative and supportive environment.
Other:
- Participate in staff development activities to support professional growth.
- Follows agency Compliance Plan and Standards of Conduct
- Support and participate in the agency's Continuous Quality Improvement activities.

Education and Experience
- Associate's or Bachelor's degree in Nursing from an accredited institution.
- Minimum of 2 years of nursing experience, preferably in behavioral health, substance use, or community health settings
- Licensure: Current and valid RN license in New York State.
Certifications (Preferred but not required):
- CPR and Basic Life Support (BLS) certification.
- Behavioral Health Nursing Certification or related specialty certifications (e.g., psychiatric nursing, addictions nursing).
Travel:
- Occasional travel may be required for community outreach, meetings, or training and engagement events when necessary.
