Job Title: Registered Nurse
Job Specialty: Discharge Planner/Case Manager
Job Duration: 13 weeks
Shift: Day, 5x8-Hour (08:00 - 16:30)
Guaranteed Hours: Not specified
Experience: Minimum 2 years
License: DC RN Licensure (pending with proof of application accepted) or LMSW/LGSW/LCSW
Certifications: BLS
Must-Have: Completion of at least 1 travel assignment
Job Description:
- Work as a Discharge Planner Case Manager with shifts from 8:00 AM to 4:30 PM, five days a week.
- Participate in a weekend rotation, working every 5th or 6th weekend; holiday coverage is needed.
- No on-call or floating requirements.
- Accept either RN Case Manager/Discharge Planner or Licensed Social Worker with eligibility for LMSW/LGSW/LCSW.
- Perform discharge planning duties including admission criteria review, benefits eligibility, care coordination, conducting needs assessments, and ordering DME.
- Experience in home health, hospice placement, LTAC, nursing home placement, rehab placement, and acute hospital experience is required.
- Prior charge experience preferred.
- Regulatory skills required include CMS, HIPAA, JCC; workers compensation knowledge preferred.
- Facility care includes geriatric, ICU, heart and vascular, general surgery, general medicine, MICU patients.
- Must adhere to wearing business casual attire, maintaining a professional appearance. Scrubs are not allowed.
- All employees must have the flu vaccine.
- Charting is done using Cerner system.
