Job Title: Registered Nurse

Guaranteed Hours: No
Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience
License: Must have license in hand
Certifications: CPR, BLS
Must-Have: Proficient in HomeCare HomeBase EMR and OASIS, used within the last 6 months or required retraining
Job Description:
- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical/Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Maintain reliable transportation with a current driver's license and agency-required liability insurance.
- Serve 5-10 patients per day, averaging 6-8 productivity points.
- Handle various cases including Start of Care, Wound Care, and Follow-up Visits.
- Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed using the IRS standard rate.
- Participate in on-call rotations, generally one night per week and one weekend per month.
- Complete all documentation within 24 hours of patient appointments.
- Be adaptable to floating to nearby clinics if patient census is low to meet financial needs.
- Comply with dress code requirements and maintain proficiency in required electronic medical record systems.
