Job Title: Registered Nurse

Shift: Day, 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: No
Experience: Minimum 1 full year of adult home health or hospice experience
License: Must have license in hand
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase (HCHB) and OASIS; Must have a vehicle, driver's license, and auto insurance
Job Description:
- Provides, coordinates, and directs the provision of home nursing care according to physician orders based on agency policies and procedures.
- Applies the nursing process competently to deliver patient care.
- Reports to the Clinical Manager and directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conducts home visits with periodic office meetings.
- Manages an average of 5-8 patients per day.
- Responsible for various types of cases, including start of care, wound care, follow-up visits, etc.
- Must be willing to travel up to 75 miles from the home clinic.
- Compensation for mileage follows IRS standard rates from patient home to patient home.
- Expected to achieve weekly productivity points equivalent to 30 points, translating to 40 hours per week.
- Documentation must be completed within 24 hours of the patient appointment.
- Open to potential on-call rotations and weekend requirements.
- Orientation and compliance will include a retraining course if the required systems have not been used within the last six months.
