Job Title: Registered Nurse

Experience: Minimum 1 full year in Adult Home Health or Hospice
License: Must have current license
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase (HCHB) and OASIS

Job Description:
- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
- Conduct home visits and attend periodic office meetings; reliable transportation required.
- Responsible for travel up to 75 miles from the assigned care center.
- Manage an average of 5-8 patients per day.
- Participate in start of care, wound care, and follow-up visits.
- Complete all required patient documentation within 24 hours of appointments.
- Participate in weekend and on-call rotations, as needed.
- Open to traveling and providing care across the designated territory.
- Ensure all assignments are accepted, regardless of the distance between appointments.
- Ability to float to nearby clinics to pick up additional patients if necessary.
