Job Title: Registered Nurse
Job Specialty: Home Health

Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: None
Experience: Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience, depending on the specialty
License: Must have license in hand, no exceptions
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase and OASIS, used within the last 6 months or completion of a retraining course
Job Description:
- Coordinate and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process
- Responsible to Clinical Manager; directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Perform home visits and attend periodic office meetings
- Must have reliable transportation, a current driver's license, and agency-required liability insurance
- Handle an average of 5-8 patients per day
- Manage various types of cases, including start of care, wound care, and follow-up visits
- Required to travel up to 75 miles from the home clinic, mileage reimbursed at IRS Standard rate
- Must complete all documentation within 24 hours of the patient appointment; delayed or incomplete documentation may lead to cancellation
- On-call duties vary by location and profession, to be discussed during the interview
- Local applicants are accepted at the same rate
- Must be open to travel between 150-200 miles per week and cover VA Beach and Norfolk areas
