Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on specialty
License:
Must have a valid nursing license in hand
Certifications:
CPR, BLS required
Must-Have:
Proficiency in OASIS within the last 6 months, or willingness to complete a retraining course
Proficiency in HomeCare HomeBase EMR within the last 6 months, or willingness to complete a retraining course
Reliable transportation, valid driver's license, and current auto insurance
Comfort with traveling up to 75 miles from the home clinic
Ability to complete documentation within 24 hours of patient appointment
Willingness to accept every patient on schedule regardless of distance between appointments
Openness to on-call rotations
Local candidates accepted at the same rate
Must be comfortable with one weekend rotation per month
Job Description:
- Provide, coordinate, and direct the application of home nursing care according to physician's orders, based on agency policies and procedures
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Make home visits and attend periodic office meetings
- Maintain weekly productivity expectations of 30 points, equating to roughly 40 hours per week
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Travelers must have a fully stocked vehicle and be prepared for various length drives between patient homes
- Must be proficient in required EMR systems and maintain compliant documentation practices
