Job Title: Registered Nurse I
Job Specialty: Home Health

Experience: Minimum 1 full year of Adult Home Health or Hospice experience required, depending on the specialty
License: Must have a valid nursing license in hand
Certifications: CPR, BLS
Must-Have:
- Proficiency with HomeCare HomeBase (HCHB) and OASIS
- Reliable transportation, valid driver's license, and agency-required liability insurance
- Comfortable traveling up to 75 miles for patient home visits
- Ability to accept all patient assignments regardless of distance
- Completion of documentation within 24 hours of the appointment
Job Description:
- Provide, coordinate, and direct the provision of home nursing care according to physician's orders and agency policies and procedures
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Travel to home visits and attend periodic office meetings
- Manage an average of 5-8 patients per day, with approximately 30 points of productivity per week
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Position covers various territories; willingness to travel between patient homes within a designated mileage radius
- All documentation must be complete and timely to ensure effective billing and follow-up patient care
- Compliance with all job responsibilities and patient care standards is expected
