Job Title: Registered Nurse

Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: No
Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience. Recent specialty experience required.
License: Must have a valid nursing license.
Certifications: CPR and BLS required.
Must-Have: Proficiency in OASIS and HomeCare HomeBase (HCHB) systems, used within the last 6 months, or willingness to complete a retraining course. Reliable transportation, a current driver's license, and agency-required liability insurance. Willingness to travel up to 75 miles from the home clinic.
Job Description:
- Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Average 5-8 patients per day, with productivity expectations of 30 points per week.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Complete all documentation within 24 hours of the patient appointment.
- Be open to traveling from patient home to patient home, excluding commute to and from the first and last patient.
- Perform duties efficiently to meet the weekly productivity targets.
- Float to another nearby clinic if necessary to pick up additional patients.
Please ensure awareness that this is a home health assignment requiring daily travel to patients' homes.
