Experience: Minimum of 1 year in Home Health Care

License: Active RN license in the state

Certifications: BLS certified

Must-Have: Ability to perform OASIS visits

Job Description:

- Provide quality nursing care to patients in a home health setting.
- Participate in a weekend rotation, usually 2 to 3 days per month.
- Optional call assignments available.
- Ensure accurate and timely completion of timecards.
- Collaborate with interdisciplinary teams for patient care planning.
- Maintain up-to-date patient records and care plans.
- Educate patients and their families on care plans and health conditions.
- Uphold professional standards and maintain compliance with all regulations.
