Job Title: Registered Nurse
Job Specialty: Hospice
Job Duration: 13 weeks
Shift: Day, 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum of 1 full year of Adult Home Health or Hospice experience
License: Must have a license in hand
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase (HCHB) and OASIS
Job Description:
- Provides, coordinates, and directs home nursing care according to physicians' orders based on agency policies and procedures.
- Responsible to the Clinical Manager.
- Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conducts home visits with reliable transportation and holds a current driver's license and agency-required liability insurance.
- Manages an average of 5-8 patients per day.
- Completes documentation and travels within assigned territory, covering up to 75 miles.
- Handles various types of cases, including Start of Care, Wound Care, and Follow-up Visits.
- Attends periodic office meetings.
- Accepts assignments that equate to roughly 6-8 points per day, with a weekly target of 30 points.
- Must be proficient in documentation and complete it within 24 hours of patient interaction.
- Must be open to on-call rotations and weekend requirements as needed.
- Local candidates will be accepted at the same rate.
- Responsible for driving to patients' homes daily and attending weekly meetings at the clinic.
- Must be open to travel up to 75 miles from the home clinic and be comfortable with longer drives between patient appointments.
- Contractor is expected to accept all patient assignments scheduled.
