Job Description
Job Title: Registered Nurse I
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day, 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: No guaranteed hours
Experience: Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience
License: Must have a license in hand
Certifications: CPR, BLS
Must-Have: Proficiency in OASIS and HomeCare HomeBase (HCHB), used within the last 6 months
Job Description:
- Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures through the competent application of the nursing process.
- Responsible to the Clinical Manager.
- Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conducts home visits and attends periodic office meetings.
- Must have reliable transportation, a current driver's license, and agency-required liability insurance.
- Assigned an average of 5-8 patients per day, equivalent to roughly 6-8 points.
- Types of cases include Start of Care, Wound Care, and Follow-up Visits.
- Required to travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS Standard rate.
- No on-call requirement; weekends vary per location.
- Responsible for driving to patients' homes daily and attending weekly meetings at the clinic.
- All documentation must be completed within 24 hours of the patient appointment.
- Expected to hit weekly productivity points of 30, equating to 40 hours per week.
- If not meeting financial needs due to low patient census, may discuss floating to a nearby clinic to pick up hours.
- Must be open to traveling and accepting every patient on the schedule.
- Local candidates accepted at the same rate; not open to pending licenses.
