Job Title: Registered Nurse
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year Hospice experience
License: Must have a valid nursing license in hand
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase (HCHB) and OASIS within the last 6 months
Job Description:
- Provide, coordinate, and direct home nursing care according to physician orders based on agency policies and procedures through the competent application of the nursing process.
- Responsible to the Clinical Manager and directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings. Must have reliable transportation, a current driver's license, and required liability insurance.
- Expected to handle an average of 5-8 patients per day, equating to approximately 6-8 points.
- Handle cases such as Start of Care, Wound Care, Follow-up Visits, etc.
- Must be open to weekend requirements and on-call rotations, which vary by location and profession. Discuss during the interview.
- Be willing to travel up to a 75-mile radius from the home clinic. Mileage reimbursed at the IRS standard rate; compensated only from patient home to patient home. Commute to and from the first and last patient is not billable.
- Possess proficiency with OASIS documentation and complete all documentation within 24 hours of the appointment with the patient.
- Accept every patient on the schedule regardless of travel distance between appointments; declining patients may result in a contract cancellation.
- Float to another nearby clinic to pick up patients if not receiving enough patients to meet financial needs.
- Dress Code: Scrubs
- Charting: Must be proficient in HomeCare HomeBase (HCHB).
