Job Title: Registered Nurse (RN)
Job Specialty: Home Care
Job Duration: 13 weeks
Shift: Day shift, 8:30 AM - 5:00 PM, Every Other Weekend Required, No Holidays, No On-Call Required
Guaranteed Hours: 40 hours per week
Experience: Minimum two years of relevant nursing experience with acute care/hospital experience preferred
License: NJ State Professional Registered Nurse license or multi-state compact license
Certifications: BLS - AHA
Must-Have: Homecare experience with OASIS; familiarity with EPIC
Job Description:
- Provide professional nursing care to patients in their homes through assessment, development, implementation, and evaluation of nursing care needs on an individual and continuous basis.
- Ensure compliance with physician orders in a timely manner and maintain visit frequency according to those orders.
- Document patient care in compliance with physician orders and track progress toward discharge goals.
- Perform assessments, planning, intervention, and evaluation of patient care following policies, procedures, and standards of care.
- Safeguard the delivery of safe patient care.
- Utilize the interdisciplinary team and accurately document all activities.
- Apply clinical judgment and critical thinking consistent with professional standards of care and ethical codes, ensuring safe and effective patient outcomes.
- Record clinical decisions appropriately through the designated system.
- Identify appropriate payors and comply with necessary authorizations for self and other disciplines.
- Maintain visit frequency within authorized range and utilize secure messaging to coordinate with insurance nurses for all authorizations.
- Ensure documentation of assessment, interventions, and planning meets organizational timelines.
- Complete nursing assessment, medication reconciliation, and collaborate with physicians on the Plan of Care.
- Update the care plan within two weeks of any change in patient status.
- Implement specific teaching strategies to meet established goals on the care plan and ensure documentation reflects the applied teaching strategies.
- Report patient care issues to management or appropriate nursing personnel and/or physicians promptly, providing possible solutions.
- Use management as a resource appropriately.
- Act as a liaison among physicians, clinical managers, and other healthcare team members to ensure effective coordination and resource referrals.
- Manage the coordination of services to meet client needs effectively.
- Communicate pertinent patient information and the Plan of Care during transfers of patient care loads, either daily or when taking vacation/time-off.
- Complete routine assignments accurately, consistently, and independently in a timely manner without needing supervision.
- Meet productivity requirements consistently.
- Supervise para-professional home health aide staff clinically.
- Complete supervisory visits within the regulatory time frame and document these visits accurately.
- Participate in evaluating home health aide staff.
- Maintain awareness of new or updated standards and incorporate them into nursing practice.
- Ensure documentation in the medical chart reflects current practice guidelines.
- Participate in other duties and/or projects as assigned.
- Lift a minimum of 50 lbs., push and pull a minimum of 50 lbs., and stand for a minimum of 8 hours a day.
