Job Title: Registered Nurse (RN)

Shift: Days - 8 hours, Monday to Friday, 9 AM - 5 PM, every 4th weekend on call

Experience: Minimum of 12 months of hospice experience in home settings within the past 3 years. Must have experience in case management and admissions.

License: Valid RN License

Certifications: Not specified

Must-Have: Solid experience with Homecare Homebase (HCHB) software

Job Description:
- Manage a caseload of 14-18 patients
- Complete patient admissions efficiently
- Provide compassionate hospice care in home settings
- Collaborate with a multidisciplinary team to ensure optimal patient care
- Work every 4th weekend on call
- Belong to the Blue Color team for territory management (refer to dashboard map for details)
