Responsibilities:
Clinical Care & Coordination
- Deliver comprehensive nursing care to clients, including assessment, planning, implementation, and evaluation of care plans.
- Collaborate with multidisciplinary teams to coordinate client care, ensuring that services are delivered effectively and efficiently.
- Administer and monitor medications, provide client education on medication usage, and manage medication orders and documentation.
Leadership & Supervision
- Supervise and mentor nursing staff, providing guidance, training, and delivering performance evaluations.
- Ensure adherence to agency policies and regulatory standards, and participate in the development and implementation of nursing protocols and procedures.
- Lead quality improvement initiatives and contribute to the development of best practices in clinical care.
Client & Family Education
- Provide education and support to clients and their families regarding health conditions, treatment plans, and self-care strategies.
- Act as an advocate for clients, addressing concerns and ensuring their needs are met throughout the care process.
Documentation & Compliance:
- Maintain accurate and up-to-date client records, including assessments, care plans, and progress notes.
- Ensure compliance with New York State regulations and standards for nursing practice and behavioral health services.
- Prepare and submit required reports related to client care, staff performance, and clinical outcomes.
Collaboration & Communication
- Work closely with other healthcare professionals, including physicians, social workers, and therapists, to provide integrated care.
- Communicate effectively with clients, families, and staff, fostering a collaborative and supportive environment.
Other:
- Participate in staff development activities to support professional growth.
- Follows agency Compliance Plan and Standards of Conduct
- Support and participate in Continuous Quality Improvement activities.
- Other duties as assigned.
Education and Experience
- Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing or a related field preferred.
- Minimum of 5 years of experience as a Registered Nurse, with at least 2 years in a supervisory or leadership role within a behavioral health or community health setting.
- Licensure: Current and valid RN license in New York State.
- Certifications: Advanced practice certifications (e.g., Psychiatric-Mental Health Nurse Practitioner) or relevant specialty certifications preferred.
Travel:
- Occasional travel may be required for community outreach, meetings, or training and engagement events when necessary.
