Hiring Range : $35.82-$46.57

The actual compensation for this position will be determined based on experience and other factors permitted by law.

Responsibilities:

Clinical Care & Coordination
  • Deliver comprehensive nursing care to clients, including assessment, planning, implementation, and evaluation of care plans.
  • Collaborate with multidisciplinary teams to coordinate client care, ensuring that services are delivered effectively and efficiently.
  • Administer and monitor medications, provide client education on medication usage, and manage medication orders and documentation.

Leadership & Supervision
  • Supervise and mentor nursing staff, providing guidance, training, and delivering performance evaluations.
  • Ensure adherence to agency policies and regulatory standards, and participate in the development and implementation of nursing protocols and procedures.
  • Lead quality improvement initiatives and contribute to the development of best practices in clinical care.

Client & Family Education
  • Provide education and support to clients and their families regarding health conditions, treatment plans, and self-care strategies.
  • Act as an advocate for clients, addressing concerns and ensuring their needs are met throughout the care process.

Documentation & Compliance:
  • Maintain accurate and up-to-date client records, including assessments, care plans, and progress notes.
  • Ensure compliance with New York State regulations and standards for nursing practice and behavioral health services.
  • Prepare and submit required reports related to client care, staff performance, and clinical outcomes.

Collaboration & Communication
  • Work closely with other healthcare professionals, including physicians, social workers, and therapists, to provide integrated care.
  • Communicate effectively with clients, families, and staff, fostering a collaborative and supportive environment.

Other:
  • Participate in staff development activities to support professional growth.
  • Follows agency Compliance Plan and Standards of Conduct
  • Support and participate in Continuous Quality Improvement activities.
  • Other duties as assigned.

Education and Experience
  • Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing or a related field preferred.
  • Minimum of 5 years of experience as a Registered Nurse, with at least 2 years in a supervisory or leadership role within a behavioral health or community health setting.
  • Licensure: Current and valid RN license in New York State.
  • Certifications: Advanced practice certifications (e.g., Psychiatric-Mental Health Nurse Practitioner) or relevant specialty certifications preferred.

Travel:
  • Occasional travel may be required for community outreach, meetings, or training and engagement events when necessary.
