POSITION TITLE: Registered Nurse

STATUS: Exempt, Full-Time

LOCATION: Jackson, California

SALARY RANGE: $94,390-$113,152, Annually

POSITION SUMMARY:

As a member of the health care team, the incumbent serves as a Registered Nurse performing nursing duties for the care of patients (ambulatory care and emergency cases). The registered nurse will be the key site coordination liaison, and will abide by and implement departmental manuals, standards of practice, and other established protocols. The incumbent will also be responsible for program administration manuals, contract compliance requirements for specific services, nursing practice regulations and other pertinent information. Employee must work in a positive and respectful manner with fellow employees.

Requires an awareness and keen appreciation of Indian traditions, customs and socioeconomic needs; an ability to meet and deal effectively with Indian Health Organizations, consisting of numerous tribes with varied cultures which requires tact, courtesy, discretion, resourcefulness, and good judgment in handling functions of a sensitive nature.

PRIMARY RESPONSIBILITIES:
  • Responsible for selecting, site orienting, training, counseling and disciplining of medical support staff. Supervise and/or coordinate clinic personnel to maintain effective and efficient patient flow through the clinic.
  • Provide enhanced care management services to MACT patients including: reaching out to patients to initiate care, conduct ongoing outreach and engagement with patients, conduct comprehensive assessments, develop and update care plans, and support patients and their authorized care representatives in achieving patient health goals.
  • Coordinate patient care management services with providers including the management of referrals and follow-ups.
  • Provide in person support at appointments and coordinate patient services after hospital discharge.
  • Assist medical providers in the treatment of patients to include but not limited to ambulatory care procedures, e.g. pelvic exams, suturing lacerations, vision, hearing tests, and assessments for Medicare and annual physicals
  • Provide patient education.
  • Adhere to protocols for nurse-initiated procedures (pediatrics immunizations, flu vaccinations, head lice, etc.)
  • Triage patients as outlined in the Ambulatory Clinic Nursing Manual.
  • Perform patient care as directed by the medical provider, and according to organization protocols.
  • Perform basic lab tests as outlined in the Laboratory Services Manual.
  • Ensures that all referrals for outside medical services are done in a timely manner.
  • Document all nursing care services provided in patient medical electronic records.
  • Utilize medical equipment; i.e., peak flow meters, tympanogram, EKG machines, autoclave, audiometer, etc in an appropriate manner.
  • Address emergency situation in a controlled and professional manner.
  • Complete other nursing tasks as assigned by Site Manager or medical provider.
  • Have primary responsibility in assuring efficient patient flow during daily operation.
  • Refer to Ambulatory Care Nursing Manual, Program Administration Manual, and established protocols as necessary to comply with this organization's standards of practice.
  • Direct and perform front and back office duties:
  • Assist with scheduling patient referrals.
  • Assist provider with appropriate paperwork (CHPD forms, referrals, PPC forms, etc.) for each patient chart.
  • Maintain current knowledge of front desk procedures as required by the Finance Dept.
  • Assist Medical Assistants as necessary to chart medical reports and records, so that this information is available to the provider.
  • Actively participate in the Medical Department's Performance Improvement activities.
  • Keep all tracking systems up-to-date. Tract pediatric immunization rates; report each quarter's statistics to Outreach Services Coordinator.
  • Complete other administrative tasks as assigned by Site Manager, Medical Director, and Executive Director.
  • Train medical support staff as needed.
  • Must adhere to the MACT Health Board, Inc. Corporate Code of Conduct.
  • Responsible for compliance of all State/Federal and outside agencies regulations and reporting related to medications and immunizations.
  • Must complete EHR training.
  • Comply with all MACT policies and procedures.
  • Other duties as assigned.

  • MINIMUM QUALIFICATIONS:
  • Current California license as Registered Nurse.
  • Current CPR certification.
  • Availability to work with a variable schedule.

  • MANDATORY REQUIREMENTS:
  • Must pass Occupational Health Screening.
  • Must possess a valid form of identification as listed on the U.S. Department of Justice I-9 form, verified using the E Verify system.
  • Clean DMV driving record and MACT auto insurance driving eligibility (determined by auto insurance carrier).

  • DESIRED ABILITIES:
  • Commitment to improve the health status of Indian people.
  • One year supervisory experience in ambulatory clinic or equivalent.
  • Familiarity with computerized medical management system desirable.
  • Capacity to use oral and written language effectively.
  • Professionalism in dealing with the public and with co-workers to promote good relations, encourage participation in program services and to enhance the concept of teamwork toward meeting patient needs.
  • One year acute or ambulatory care experience.
