Transition Coordinators assess patients' needs in the context of the whole person including their life situation and determinants of health. Critical thinking and accurate interpretation of complex information from a variety of sources are essential to the clinical decision-making required for the determination of appropriate continuing care living options. The role of the Transition Coordinator is highly complex and includes comprehensive assessment, planning and coordination of care, as well as ongoing communication with other health-care providers across the continuum of care. Major responsibilities include: completing comprehensive electronic needs assessments, determining urgency/priority, developing and implementing appropriate service plans, coordinating service delivery across programs/settings, liaising with Continuing Care staff and other community resources, providing information and support to clients, families, and caregivers. The Transition Coordinator works within a multi-disciplinary team of health professionals and is responsible for appropriate referral to other programs/services, appropriate documentation, participation in conferences and team meetings, process, and outcome evaluation and professional development activities. Work tasks and charting is computer based so there are extensive repetitive motion tasks. Ability to perform all assigned tasks and complete physical nursing assessments as required, including but not limited to: lifting; physical exertion including bending/walking /sitting or standing for long periods of time. Must be able to participate in hands on care when required.

The Registered Nurse (RN) participates in nursing assessments and interventions, care planning, implementing prescribed treatments, administering medications, evaluating treatments, and monitoring/documenting treatment progress and patient/resident response. Acting as an advocate, coordinator and liaison for the patient/resident, the RN identifies needs and sets priorities in collaboration with the multidisciplinary team. The RN facilitates patient/resident and family education and discharge planning, and may be required to act in a charge nurse capacity. The RN is responsible for the provision of care in accordance with established standards, and the Mission, Vision, Values, policies and procedures of Covenant Health. This position is covered by the terms and conditions of the UNA Collective Agreement.
Graduate of a Registered Nurse program or recognized equivalent required. Active registration with the College of Registered Nurses of Alberta (CRNA) required. Basic Life Support Healthcare Provider (Level C) required. Additional Required Qualifications:
Minimum of one (1) year Charge/Triage Nurse experience is required. Three years of acute care nursing experience, within the last five years, specifically in adult/geriatric patient populations within Medicine, Surgery, Critical care and/or ER, required. Demonstrated time management and prioritization skills are required. Proven conflict resolution and strong communication skills are required. Must be able to work independently and within a team environment. Ability to problem solve and demonstrate critical thinking skills is required. Demonstrated ability to provide comprehensive assessments to individuals within their physical and social environments is required. Must be able to cope with high levels of stress, fluctuating priorities, and time sensitive deadlines. Must be able to tolerate long periods of sitting or standing as well as repetitive movements. Must be able to lift and carry 10-15kg regularly. Must be able to participate in hands on care of patients (ie: transfers, bariatric clients, bending, twisting, and walking).
Preferred Qualifications:
Minimum of two years' experience within the community is preferred (Home Living and/or Continuing Care). Previous Transition Coordinator experience is highly preferred. Knowledge of computer applications such as Microsoft Word, Pathways, RAI Tools, Meditech and e-People is preferred and an asset.
