Description:
This role involves utilizing a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet a patient's health needs.
Good communication and available resources will be used to promote quality, cost-effective outcomes for 18-22 patients.
The Care Manager will meet patient needs, manage length of stay, and promote efficient utilization of resources.
Determining medical necessity and appropriateness of admission will be conducted using established screening methods.
