SALARY RANGE: $80-90k/yr (DOE, Negotiable)

SHIFT: Full Time, M-F (On-Call triage as needed)

We provide Memory Care and Assisted Living services in small community settings fostering strong interpersonal staff and resident relationships.

POSITION DETAILS:

This position will ensure that the health and safety of all clients are met through assessments of the clients, development & implementation of appropriate plans of care, supervision of clients' services, supervision of home care staff and on-going training of home care staff, and other duties as assigned.

This role is very independent in nature as you will be the sole/primary RN on-site, overseeing the entire community of approximately 30 residents in Memory Care & Assisted Living.

RESPONSIBILITIES:
  • Serve as the primary Nurse on-site, responsible for overseeing the cares of all Residents and supervision of unlicensed staff.
  • Partner with Housing Director in overall operations, supervision, and execution of care throughout the community.
  • Complete initial, face-to-face comprehensive assessments of prospective/new clients and develop a proposed service plan.
  • Monitor and reassess clients' needs and care plans as necessary.
  • Responsible for educating clients and clients' representatives about advance directives, medications, safety of side rails or assistive devices, potential hazards in the home and other relevant information.
  • Assist communities to maintain an active and functional Quality Assurance Community Program to include QA incident reviews, self-surveys, resident satisfaction surveys, MSL complaint program, identification of deficiencies, development and implementation of plans of improvement and to assist with any plan of correction.
  • Lead staff training programs and assures staff educational requirements are met.
  • Facilitates coordination of client's home care services with other service providers, such as hospice, and transitions to the hospital or nursing home.
  • Assures that proper systems, policies and procedures are developed and implemented correctly by licensed and unlicensed staff regarding the storage, set up, administration and disposal of medications, including systems to control controlled medications.
  • Assures that clients' medication records are up to date, that medication administration procedures are followed properly, investigates any errors and issues with staff, and implements corrective action plans as needed.
  • Provides direct supervision of new licensed and unlicensed staff within 30 days of date the staff person begins work and thereafter provides direct supervision on a schedule consistent with the staff person's experience and performance to assure that procedures are followed accurately and that the client's needs are being met.
  • Directly supervises LPNs' and unlicensed staff's performance of delegated nursing services. Conducts evaluation of staff performance of delegated nursing services in coordination with the Assisted Living Director and Regional Director of Nursing through appropriate documentation and consistent with agency personnel policies for performance reviews.
  • Addresses client and family concerns or complaints to the extent possible and follows up with appropriate staff to resolve the issue. Document complaints and report complaints to the Assisted Living Director


Job Requirements

QUALIFICATIONS:
  • Must be a graduate of a school of Nursing and must be registered and licensed with the State of Minnesota as a RN.
  • Must maintain current unencumbered license with the Board of Nursing.
  • At least 1 year experience in Home Care or Long Term Care.
  • Experience with assessing and executing care of residents as they approach Hospice and through end of life care.
  • Management, training & oversite of unlicensed caregiving personnel.
  • Must have current knowledge of best practices for the elderly and residents with Dementia.
  • Must be able to communicate effectively with staff, residents, families, community management officials, state regulatory agencies and agency staff, physicians, hospitals and the general public.
  • Familiarity with PointClickCare highly desire
  • Previous experience in the Supervision of Care staff highly desired
  • Previous experience with On-Call for triage care staff support highly desired
  • Must be available for on-call medical questions and/or emergencies via phone as assigned. On-call duty may require the RN to come to the client's home to handle a medical emergency.
