Reporting to the Director or assigned Supervisor, facilitates a positive, collaborative work environment with an interdisciplinary team approach. The Care Coordinator utilizes the nursing process in the delivery of patient care including assessment, planning, implementation and evaluation of patient outcomes. Provides department supervision and oversees the staff scheduling functions to effectively utilize resources, and modifies a patient's plan of care as indicated. Resource to the healthcare team collaborating to achieve optimal clinical, financial and satisfaction outcomes in primary and specialty care. Assesses patients, facilitates plan of care, implements and coordinates care progression, monitors and evaluates options and services to meet an individual's health needs. - Hourly, Days and Evenings
- Weekend as needed
What you will need:
- Successful completion of a BSN from an accredited school of nursing
- Current licensure to practice nursing in the State of Illinois
- Current CPR certification from the American Heart Association
