Job Description
Job Title: RN Care Manager
Job Specialty: Registered Nurse
Job Duration: 13 weeks
Shift: Monday to Friday, 8am-5pm & Weekend rotation with call
Guaranteed Hours: Not specified
Experience: Minimum of 3 years in a clinical care setting with knowledge of discharge planning, case management, or utilization review processes. Must have 1 year of travel experience.
License: A valid, current, full, and unrestricted Professional Nursing License to practice nursing in any state or territory.
Certifications: Current Basic Life Support (BLS) certification from the American Heart Association required throughout employment.
Must-Have:
- Associates degree in Nursing; must obtain bachelor's in nursing within 2 years of hire.
- Demonstrated knowledge of electronic health record systems.
Job Description:
- Serve as an Outpatient/Inpatient Social Worker/Registered Nurse Case Manager.
- Responsible for supervising and providing supportive contact for high-risk patients.
- Design and manage a continuum of care to empower clients to achieve outcomes and self-sufficiency.
- Responsibilities include assessment, service planning, monitoring progress, and responding to client needs.
- Link clients with community social service providers, healthcare providers, substance abuse, and mental health providers.
- Conduct utilization reviews for inpatient and outpatient services, assessing the medical necessity of referrals to specialty providers.
- Collaborate with a multidisciplinary team to enhance patient care, self-care, and quality of life while containing costs.
- Work closely with various healthcare providers to ensure appropriate resource implementation.
- Responsible to the Director of Case Management/Care Coordination.
Additional Information:
- Orientation conducted every two weeks.
- No religious exemptions accepted for COVID; medical exemptions require approval.
- Housing application must be submitted 2 weeks prior to start date.
- EMR system used: Allscripts
- Holiday coverage required.
