Job Summary

This position serves as a Traveling Registered Nurse Case Manager in a facility in Taos, NM. A minimum twelve (12) month commitment is preferred. Options for additional consecutive twelve (12) month commitments do typically exist but are based on client needs. The purpose of this position is to provide evaluation and implementation of comprehensive healthcare plans for individual patients and act as a liaison between patients, families, and healthcare providers.

Compensation:

This pay band reflects Chloeta's good faith estimate of the range it expects to offer; Chloeta reserves the right to alter this range if business circumstances change.

  • $48.00 - $55.00 per hour

Duties/Responsibilities

  • Develops, implements, and evaluates care plans


  • Promotes health care outcomes with currently accepted clinical practice guidelines


  • Provides patient education, advice, and information on health assessment, disease processes, medications and treatment plans


  • Integrates patient and family education into the delivery of care. Educational activities will recognize the age, gender, race, ethnicity, diversity, and cultural norms of the individual patient and incorporate the patient and family's ability to understand and participate in the plan of care


  • Assesses patient needs using established clinical guidelines, protocols, and pathways


  • Initiates therapeutic interventions and administers medications as prescribed by medical providers


  • Provides appropriate follow-up as directed or per established guidelines Assists with obtaining lab results, medical tests, discharge summaries, and other information within the hospital and other facilities and assists with arranging appointments, procedures, and discharge planning.


  • Uses appropriate medical supplies and/or equipment for the assigned clinic, such as 12-lead EKGs, fetal monitor, pulse oximeter, etc., and collects patient specimens, including blood draw, if needed.


  • Processes lab specimens for transport or perform tests, such as waived tests, to obtain results.

Required Education/Experience

  • Required: Current, valid, unrestricted nursing license in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States throughout the term of employment.


  • Required: A bachelor's degree in nursing from an accredited school in the U.S.A.


  • Required: One year of nursing experience that includes nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques.


  • Required: Current Basic Life Support (BLS) is mandatory.


  • Required: Provide three (3) professional references at the time of application.


  • Required: Must be able to pass a federal background check.


  • Preferred: Pediatric Advanced Life Support (PALS), Newborn Life Support (NLS), Advanced Trauma Care Nurse (ATCN) or Trauma Nurse Critical Care (TNCC), and Advanced Cardiac Life Support (ACLS) are recommended for each appropriate duty section at the time of selection for contract award. The contractor shall ensure that the contract nurse's certifications remain valid throughout the term of this contract.


Physical Requirements

  • Must be able to withstand physical and emotional stress.


  • Must be able to stand and move for long periods of time.


  • Able to frequently lift at least 50 lbs., reach, pull, push, bend, stoop, and kneel.


  • At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.


  • At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

Pre-employment Requirements

Due to the nature of Chloeta's services, this position is contingent upon completing with a satisfactory outcome some or all of the following pre-employment background screenings: government security clearance, federal and state criminal background checks, drug test (urinalysis), Motor Vehicle Record (MVR) review, educational history, employment history, and/or credit check. In accordance with the Fair Credit Reporting Act (FCRA), pre-employment background screenings will require the employee's written authorization and will occur after acceptance of an offer of employment and prior to commencing work. Additionally, employees are required to complete the federal I-9/E-Verify process.
