Job Description:
- Engage the homeless in various locations such as city streets, subways, and parks to conduct medical, mental health, and safety assessments.
- Encourage the homeless population to seek shelter in the city.
- Conduct field assessments with crisis coordinators and outreach teams, offering services and building trust with individuals.
- Provide necessary services by determining if individuals have mental illnesses or medical conditions that require immediate observation, care, and treatment.
- Work as part of a team to respond to different locations and conduct street-level mental crisis assessments.
- Evaluate mental health conditions to determine if individuals are at risk of causing harm to themselves or others.
- Assess for homicidal or violent behavior and determine if it poses a risk to others.
- Collaborate with law enforcement to transport individuals in mental crisis to appropriate facilities if necessary.
- Coordinate with outreach teams on comprehensive care and discharge planning.
- Provide medical attention to individuals under mental hygiene laws and initiate appropriate processes for their care.
- Document and assess individuals removed during fieldwork, maintaining records accurately and promptly.
- Communicate with hospital staff and clinicians regarding assessments and recommended treatment plans.
- Obtain necessary approvals to operate as part of a mobile crisis outreach team.
